LensRefresh does not guarantee insurance coverage or reimbursement. Coverage depends on your individual vision plan, insurer, and eligibility requirements.

For eligible orders, we can provide an itemized receipt and available order details for you to submit with your insurance claim.

Before You Order

Please confirm the following directly with your insurance provider:

• Whether your LensRefresh purchase is eligible
• Your remaining coverage and reimbursement limits
• Any prescription or supporting-document requirements
• The deadline and process for submitting your claim

Customers are responsible for confirming coverage and submitting their claims. Insurance reimbursement is not guaranteed.
